We ask that you offer the following items no later than October 1st (or the midpoint of your program).

● 2 images (advertisement or photos of working group or location, if actual pictures of participants are not yet available)

● Final/current schedule (if the final schedule is not available, share the schedule you anticipate at this point!)

● Updated description of the anticipated impact on the community (What has changed? Outcomes? 500 words or less sharing what you expect to happen!)

● Overview of financial investment to date (please share a bit more about how you expect to utilize the funds)

Now that you’ve completed your program, please be sure to report to the Intentional Growth Center within 60 days of completion. We ask that you offer the following items.

● At least two (2) additional images (advertisement or photos of working group or location)

● At least three (3) Quotes/testimonies/stories sharing about your program or initiative

● Final schedule

● Final description of the impact on the community (What has changed? Outcomes? What have you learned? What might you do differently next time? 500 words or less sharing what happened!)

● Final number and reach of participants (How many participated? Where are they from?)

● Overview of financial investment (please share a bit more about how you utilized the funds)
